The program is a multi-modal system of intensive treatment with an expected length of stay determined by individual assessment. Clients participate in individual and group therapy, with specific treatment focuses based upon the client’s assessed needs. Intensive Outpatient Services are designed to provide structure and support for individuals who are in need of intensive treatment and can remain in their own living environment while receiving these services on an outpatient basis. The individual participating in this level of care will receive individual, group, and family counseling. Keep The Faith Foundation is a minority, faith-based, not-for-profit organization dedicated to the total recovery and restoration of persons suffering from the disease of addiction. We offer services such as: Vocational training and access to employment resources; Recovery / life coaching and mentoring. Our program consists primarily of individuals seeking vocational training subsequent to accomplishing recovery from substance abuse and ex- offenders seeking adult education assistance / employment resources. Since participants come to us requiring a variety of services and each individual situation is unique, they are assessed in a welcoming yet thorough manner. Keep The Faith Foundation is located on Chicago's West side and we provide services for, primarily, all communities within the Cook County district. As a faith-based organization we make use of the insights of religious traditions and the principles of social services to administer our pastoral counseling. Anixter Center's Substance Abuse Prevention Program provides the Rogers Park and West Ridge communities with the only youth mentoring program focusing on substance abuse. The goal is to support and empower youth so that they reach their full potential. Some of the life skills that are emphasized are social / emotional well-being, academic performance, decision making, identity development and positive attitudes and behaviors. A Safe Haven provides Drug and Alcohol Treatment and individual and group recovery education, programs and meetings. Based on a 12 step model, our professional mental health services benefit participants on site. A Safe Haven has 130 employees, an annual budget of $16 million, and 500 apartments in 16 sites for permanent housing. In addition, there are 1,375 beds for temporary housing in a program that includes supportive housing, meals, case management, drug and alcohol treatment, health care, job training and placement, and access to affordable housing. The housing serves homeless singles as well as single parents and their children, as A Safe Haven works to reunite families torn apart by addiction. A Safe Haven helps clients with a program based on six guiding principles. The foundation emphasizes resilience as a key to self-sufficiency, recognizes multiple long-term pathways to recovery, and empowers individuals and families to continue their healing once they become independent. It bases its practices on scientific studies of what is working for its clients. The foundation also works with military veterans back from the war in the Middle East—Brian Rowland is himself an Army veteran. The foundation also reaches out to men and women discharged from local jails and prisons. A Safe Haven’s clients get assessed at the foundation’s Center for Workforce Development, then take courses in job readiness to qualify for job training in six areas: food service, landscaping, housekeeping, security services, telemarketing, and sales. About 80 percent of those in the job training program secure job placements, and that can provide the financial foundation to support recovery, stability, and, eventually, independent living. Near North Health Service Corporation
Near North Health Service Corporation is a non-profit Federally Qualified Health Center (FQHC). It is one of the largest providers of community-based primary care in Chicago. We provide health care, social services and nutrition education to the medically indigent and uninsured residents of the Near North side (Cabrini Green), West Town, Humboldt Park, West Garfield Park, Austin, Kenwood / Oakland, Douglas and Grand Boulevard, and Uptown communities. NNHSC provides mental health services to patients who are suffering from both acute and chronic psychological / emotional conditions. We offer both medication management and supportive talk therapy to treat conditions such as anxiety, depression, stress, and emotional distress. Our philosophy reflects an approach that anticipates community needs and plans for expansions that are developed in accordance to these needs. Throughout our continuous evolution, we are happy to report that not only have our methods held up under the intense new demands of ever-changing communities, but also we have come through with shining results. Hazelden has been saving lives and restoring families from substance abuse and addiction for more than 60 years. With outpatient and residential programs in Illinois, we offer every patient the best chance of lifelong recovery. Hazelden helps individuals, families, and communities struggling with alcohol abuse, substance abuse, and drug addiction transform their lives. ReVive Center for Housing and Healing is the trusted not-for-profit organization that is committed to solving homelessness in Chicago. For almost 100 years, thousands of homeless individuals and their children have turned to CSC for expert resources, award-winning programs and a compassionate place to heal and call home. CSC is a vital resource based on Chicago’s west side, helping individuals and their families overcome chronic homelessness to gain hope and a fresh start regardless of their background. Cathedral Shelter of Chicago is a vital piece to the city’s west side, providing community assistance, life skills development and resources for thousands of families and almost a hundred permanent supportive housing clients. CSC recognizes the importance of keeping the family together, especially during difficult circumstances, and provides programs for children and individual family members in need of assistance while also supporting the family as a whole. The organization offers 28-unit permanent supportive housing, Cressey House, and experienced staff members who understand and respect the condition of addiction and chronic homelessness in Chicago. ReVive Center for Housing and Healing provides award-winning services to adult men and women through Higgins House. Higgins House’s services are free of charge to individuals with no income or insurance, or provided on a sliding fee scale. We try our best to accommodate client schedules, and deliver services in a flexible and compassionate atmosphere. Participants are also encouraged to make use of our other programs and services. Department of Psychiatry at the Cook County Health and Hospitals System. Though modest in terms of staffing, our department engages 80 providers throughout the health system in hospital, primary care, psychiatric specialty, school-based, and correctional settings. Identifying the strengths of our faculty and the needs in our community, we have developed specialized treatment areas for Aging and Geriatrics, HIV / AIDS Psychiatry, Substance Abuse, Women’s Health, Adult and Child ADHD, Developmental Child Assessments and Therapy, Child and Adult Neuropsychological Testing, Rehabilitative Psychology, Biofeedback and Pain Management. In 2011, the DOP’s Substance Abuse Service (SBIRT) was recognized for its comprehensive and caring approach of addiction by the United States’ Office of National Drug Control Policy and received accolades by community agencies for its monolingual Spanish clinic (Conexiones) bringing together bilingual Spanish providers to serve these patients and families. The Department has excelled in clinical and service research with current funding from federal agencies including HRSA, CDC, NIDA, Adolescent Trials Network, NIMH, SAMHSA, NIAAA, and the Department of Justice. Through this additional work and services, we are able to provide novel and more comprehensive care to a greater number of patients in need.
